Pay Range:
– California: $90,000–$100,000 annually
– Other states: $80,000–$90,000 annually
Shifts:
– Tuesday to Saturday, 8 AM–5 PM PST
– Sunday to Thursday, 8 AM–5 PM PST
Location:
Candidates may live in any of the following states but must be available to work West Coast hours:
Arizona, California, Idaho, Illinois, Nevada, Oregon, Texas, Utah, Washington
Position Overview – Quality Care Manager:
The Quality Care Manager (QCM) is responsible for reviewing patient assessments to ensure care is appropriate, effective, and compliant with agency standards and regulations. The QCM identifies documentation trends and communicates findings to the Quality Director to support performance improvement and staff education.
Key Responsibilities:
Clinical Documentation & Compliance
– Review documentation and care plans for accuracy and alignment with care goals and physician requirements.
– Ensure records demonstrate medical necessity, skilled care needs, and proper visit use.
– Confirm documentation supports recertification and discharge decisions.
– Approve additional orders and complete HOPE assessments promptly.
– Provide feedback on documentation issues and reinforce agency protocols.
Quality & Regulatory Oversight
– Stay current on hospice regulations, CHAP standards, and HOPE assessments.
– Collaborate with the Director of Quality and Education to enhance HOPE accuracy and patient outcomes.
– Use SHP data and alerts to drive real-time performance improvements.
Collaboration & Communication
– Maintain clear communication with care teams, leadership, and departments.
– Participate in case conferences and departmental meetings.
– Ensure adequate coverage across offices, including holiday/weekend on-call rotations.
Workflow & Operational Excellence
– Complete daily workflow tasks to avoid care delays and support timely billing.
– Demonstrate strong organization, attention to detail, and problem-solving skills.
– Follow up on outstanding tasks and adhere to agency procedures.
Qualifications:
– Graduate of an accredited nursing program.
– Active, unrestricted RN license in California required; must obtain licensure in OR, UT, ID, and NV (support provided).
– At least one year of hospice experience with knowledge of Medicare Hospice Conditions of Participation, HOPE Assessments, and Homecare Homebase.
– Strong written and verbal communication skills.
– Ability to manage stress and meet deadlines.
– Proficient in computer systems; Homecare Homebase experience preferred.
– Self-motivated and capable of working independently.
– Effective communicator with staff, leadership, and healthcare teams.
