Compensation: $56 – $60 per hour, based on experience, plus mileage reimbursement
Schedule: Full-time
Location: Orange County

Key Responsibilities:
– Initiate and update care plans based on patient needs and condition changes
– Use health assessments to determine nursing diagnoses
– Develop care plans with goals and palliative actions, involving patients and families
– Provide health education based on assessments and care plans
– Support patients with daily living activities and comfort care
– Serve as Case Manager when assigned, coordinating care for your caseload
– Communicate with physicians about patient needs and condition changes; obtain necessary orders
– Educate patients and caregivers on self-care, medications, diet, and more; offer health counseling in collaboration with the care team

Qualifications:
– Graduate of an accredited nursing program; BSN preferred
– Current Registered Nurse license in the state
– At least one year of recent acute care experience in a clinical setting
– At least one year of recent home care experience, preferably in hospice
– Valid driver’s license and reliable, insured transportation
