Benefits for Per Diem/Part-Time Team Members:
– 401(K) retirement plan

Compensation:
– $35–$45 per hour, based on experience, plus mileage reimbursement

Work Schedule:
– PRN/Per Diem as needed, with occasional Saturdays (5–15 hours per week; flexible scheduling)

Location:
– North County San Diego

Key Responsibilities:
– Deliver skilled nursing care in patients’ homes under the direction of an RN or physician
– Monitor and report changes in patient condition
– Administer prescribed medications and treatments
– Perform clinical tasks such as wound and catheter care
– Educate patients and families on health conditions and care plans
– Maintain accurate and timely documentation of patient visits
– Collaborate with the care team to ensure high-quality outcomes

Qualifications:
– Graduate of an accredited LVN/LPN program
– At least one year of experience, ideally in home health or med/surg
– Current state licensure and CPR/BLS certification
– Strong communication and problem-solving abilities
– Physically able to meet job demands
– Valid driver’s license and access to a reliable, insured vehicle
