The Care Navigator will collaborate closely with patients, providers, and the broader care team to ensure members receive the highest-quality care. Our innovative team brings together top talent from healthcare and technology to deliver modern, concierge-style support. This includes helping members navigate the healthcare system, coordinating their care, and offering guidance and education.

This position is a 3–6 month contract, with the exact length depending on evolving business needs. Candidates should be ready for some flexibility in assignment duration.

What you will do
• Provide comprehensive care navigation and access support to ensure members receive the appropriate care at the right time, place, and cost.
• Work with cross-functional teams, including virtual care and customer support, to deliver seamless navigation services.
• Educate members about their referral options so they can make informed decisions.
• Partner with providers to secure access to care through established processes, such as provider education, contract discussions, and billing inquiries.
• Identify contracting opportunities to help grow the network and improve member access.
• Build strong relationships with providers and care partners to simplify referral pathways.
• Proactively reach out to high-risk, rising-risk, and gap-in-care members to guide them through their care journeys.
• Serve as a health advocate, always prioritizing the member’s best interests.
• Monitor and evaluate care navigation strategies and adjust based on clinical judgment, evidence-based guidelines, and performance metrics.
• Share feedback with the product team to enhance the member experience.
• Consistently work to support members and simplify their overall healthcare experience.

About You
• 2+ years of work experience, ideally in a fast-growing organization.
• At least 1 year of experience in healthcare (such as social work, managed care, insurance, etc.).
• Care navigation experience is strongly preferred.
• Excellent communication skills.
• A team-oriented approach, with the ability to build strong relationships both internally and externally, including in a remote environment.
• Mission-driven, with a desire to improve the healthcare system for individuals and businesses.
• Curious and eager to learn, comfortable asking questions, and willing to engage in thoughtful, solution-focused discussions.
• Flexible, resilient, and committed to continuous learning, while also prioritizing self-care to support the team.
• Action-oriented, willing to take smart risks, make decisions with imperfect information, and push forward innovative ideas.
• Responsible and reliable, following through on commitments, owning mistakes, and building trust across the team.

$25–$30 per hour

The targeted cash compensation for this role is $25.00–$30.00 per hour for all U.S.-based remote locations. Final offers depend on a variety of factors, including experience and expertise, and may differ from the range listed.
