The Role
Synapticure is hiring a Transitions of Care Navigator (TOC Navigator) to assist patients and caregivers during important care transitions from hospitals or other facilities back into their communities. This role focuses on ensuring patients receive the necessary education, resources, and follow-up care to reduce hospital readmissions, improve health outcomes, and boost satisfaction.
As a TOC Navigator, you’ll act as a bridge between patients, caregivers, and healthcare providers—creating tailored care plans, tracking progress, and coordinating services to ensure smooth transitions. The role requires strong communication, problem-solving skills, and a deep commitment to supporting vulnerable populations through continuous care.
Key Responsibilities
– Work closely with physicians, discharge planners, social workers, and primary care providers to support safe care transitions
– Assess patients to identify medical, emotional, and social challenges to successful discharge
– Develop and execute personalized care plans based on discharge instructions and patient needs
– Educate patients and families on medications, treatments, follow-up care, and lifestyle changes
– Teach patients how to recognize symptoms that may require urgent or emergency care
– Arrange follow-up appointments with primary care providers, specialists, or community services and monitor compliance
– Conduct follow-ups after discharge via phone, telehealth, or in-person visits to track recovery and address any issues
– Serve as a communication link between patients, caregivers, and providers to ensure timely and clear care delivery
– Maintain accurate records of interactions, progress, and follow-ups in electronic health systems
– Report outcomes such as readmission rates and patient satisfaction to help improve care processes
– Collaborate with multidisciplinary teams and participate in case reviews to support continuity of care
Requirements
– Bachelor’s degree preferred
– At least 3 years of experience in transitions of care or a related healthcare setting
– Skilled in using virtual care platforms, electronic health records, and scheduling tools
– Excellent verbal, written, and organizational abilities; able to work well in diverse teams
– Experience accurately collecting and documenting clinical and demographic information
– Strong decision-making and problem-solving skills; collaborative mindset
– Flexible, resourceful, and able to thrive in a fast-changing environment
– Proven ability to build positive working relationships with patients, colleagues, and community providers
Preferred Qualifications
– Experience working with patients who have neurodegenerative diseases
– Bilingual in Spanish (spoken and written) to better support diverse patient populations
