Service Area: Orange County, CA
Schedule: Monday through Friday, 8:00 AM – 5:00 PM
Pay Range: $55–$60 per hour, based on experience
Key Responsibilities:
– Deliver skilled nursing care to hospice patients in line with state regulations and company policies
– Conduct detailed patient assessments and create personalized care plans
– Manage a caseload and coordinate care across healthcare disciplines
– Handle patient admissions, including eligibility verification and care plan initiation
– Administer prescribed medications and treatments
– Educate patients and families on managing symptoms, disease progression, and end-of-life care
– Serve as the main contact for patients, families, and healthcare providers
– Maintain accurate and timely clinical documentation
– Participate in interdisciplinary team meetings for coordinated care planning
– Provide emotional support and advocate for patient comfort and dignity
Qualifications:
– Graduate of an accredited nursing program with a current RN license
– Valid CPR certification
– At least one year of recent nursing experience; hospice or palliative care preferred
– Preferred experience in case management or hospice admissions
– Strong clinical, assessment, organizational, and communication skills
– Able to work independently and as part of a team in various care settings
– Physically capable of performing patient care duties
– Valid driver’s license, reliable insured vehicle, and compliance with driving requirements
