The Triage Registered Nurse (RN) provides support to patients and their families through safe, effective, and evidence-based telephone triage. This role combines clinical judgment with a decision-support tool to ensure consistent and safe care delivery.

Key Responsibilities:

– Perform comprehensive clinical assessments to determine the appropriate level of care and make referrals based on established protocols.
– Adhere to standard triage procedures and guidelines.
– Clearly communicate care recommendations to patients in understandable terms.
– Connect patients with internal and external resources and community services when necessary.
– Deliver care aligned with Included Health’s Core Values.
– Provide triage support for urgent prescription needs.
– Act as the main contact for emergency situations involving Included Health members.
– Engage in team meetings and contribute to quality improvement efforts.

Requirements:

– Bachelor of Science in Nursing (BSN) required.
– Must be a Registered Nurse living in a compact state and eligible for licensure in all 50 states.
– At least 2 years of triage experience (preferably phone-based), or 2+ years in an emergency department, or 4+ years in a primary care setting with triage responsibilities.
– Ability to work in Pacific Time Zone.
– Willingness to work rotating holidays and weekends (every third weekend for full-time, every other weekend for part-time).
– Strong clinical decision-making skills.
– Comfortable managing a wide range of medical issues across pediatric and adult populations.
– Experience handling complex and uncertain clinical situations.
– Excellent verbal and written communication skills, with the ability to collect clinical histories and summarize findings clearly.
– Proficient in using various computer systems, electronic medical records, and Google Workspace tools.
– Must be efficient and able to meet performance metrics without compromising care quality.
– Responsible for maintaining active nursing licensure and completing continuing education requirements.

Compensation: $32.14 – $44.00 per hour
