Position Summary:
The Quality Care Manager ensures that patient assessments are accurate and that care plans align with agency policies, procedures, and regulatory standards. Any patterns of inaccuracy identified will be reported to the Quality Director to support performance improvement and staff education initiatives.

Primary Responsibilities:

– Maintain effective communication and collaboration with the healthcare team and leadership.
– Review documentation and care plans to ensure alignment with care goals, physician evaluations, and homebound status when applicable.
– Verify that submitted documentation is accurate, complete, and supports the need for skilled care and medical necessity.
– Provide constructive feedback to clinicians regarding documentation issues or OASIS corrections, reinforcing agency protocols.
– Ensure that the number and type of visits are appropriate based on patient assessments.
– Evaluate the appropriateness of additional physician orders.
– Confirm that clinical documentation supports decisions for recertification or discharge.
– Approve and finalize the Plan of Care (Form 485) in a timely manner, ensuring it is accurate and tailored to the patient.
– Review and finalize OASIS assessments promptly, checking for inconsistencies.

Qualifications:

– Graduate of an accredited nursing program.
– Active, unrestricted Registered Nurse license.
– At least one year of home health experience with knowledge of Title XXII, Medicare Home Health Conditions of Participation, and OASIS.
– Strong written and verbal communication skills.
– Ability to manage stress and meet deadlines.
– Proficient in computer use; familiarity with Home Care Home Base is preferred.
– Self-motivated and able to work independently without direct supervision.
– Effective communicator with staff, leadership, and the healthcare team.

Salary Range: $85,000 – $105,000 annually
