In June 2023, Color partnered with the American Cancer Society (ACS) to launch a cancer screening and prevention initiative for employers and labor groups. This program combines ACS’s expertise in cancer prevention with Color’s scalable healthcare delivery. Since its launch, the program has grown to include diagnosis management, treatment, and survivorship care. Through our Virtual Cancer Clinic, we help individuals detect cancer earlier, provide empathetic and timely care during diagnosis, support survivors in returning to work and managing ongoing health needs, and address major screening gaps for high-risk cancers.
We are hiring a compassionate and experienced Nurse Practitioner to join our Cancer Survivorship Clinic, delivering ongoing, holistic care to cancer survivors in collaboration with our oncology team. This is a remote position. The ideal candidate is passionate about reducing cancer’s impact, skilled in telehealth, and experienced in caring for individuals with a history of cancer. You understand how disruptive cancer can be and are committed to making a difference.
How You’ll Make an Impact:
– Perform comprehensive survivorship evaluations, including treatment history, potential long-term effects, and individual risk factors.
– Create and manage personalized survivorship care plans based on national guidelines (e.g., NCCN, ASCO), including screening, monitoring, and preventive care.
– Help patients manage long-term effects of cancer treatment—such as fatigue, pain, cognitive changes, and hormonal issues—in coordination with oncology and primary care teams.
– Educate and counsel patients on lifestyle changes, wellness, and risk reduction to support long-term health.
– Coordinate care across various providers to ensure continuity and quality.
– Monitor patient progress and provide follow-up care via video, phone, and messaging.
– Order and interpret lab tests, imaging, and other studies as needed.
– Prescribe medications and supportive treatments within scope and protocols.
– Accurately document patient interactions and care plans in the electronic health record.
– Work under established procedures for Nurse Practitioners, in consultation with supervising oncologists as needed.
Ideal Candidate Qualifications:
– A compassionate and empathetic Nurse Practitioner passionate about supporting cancer survivors and improving outcomes.
– Master’s degree in Nursing and completion of an NP program in Adult, Family Practice, or Oncology.
– Active Nurse Practitioner certification, RN license, and national NP certification (ANCC or AANP).
– At least 5 years of clinical experience in outpatient primary care, oncology, or survivorship care; experience in managing long-term treatment effects or cancer risk is preferred.
– At least 2 years of telemedicine experience preferred.
– Valid medical license(s) in good standing; multi-state licensure is a plus.
– Strong clinical skills based on survivorship guidelines (e.g., ASCO, NCCN, ACS) with a focus on quality and patient-centered care.
– Excellent communication skills, emphasizing empathy, education, and shared decision-making.
– Collaborative team player, comfortable working with a multidisciplinary team.
– Strong organizational skills and ability to thrive in a fast-paced environment.
– Comfortable using technology such as EHRs, telehealth platforms, and messaging tools.
– Experience in health tech is a plus.
– Proficiency in medical Spanish is a significant advantage.
