We are dedicated to delivering positive health outcomes for patients nationwide, regardless of their circumstances. Our goal is to give everyone “insider access” to high-quality healthcare. At Included Health, our virtual Primary Care Providers (PCPs) help advance this mission by ensuring patients receive excellent care and fully understand the reasoning behind their medical recommendations.
We are seeking clinicians to join our team who can provide virtual medical care and professional support to patients via video and phone. Ideal candidates thrive in a collaborative, fast-paced environment and are passionate about delivering holistic, comprehensive primary care. Strong communication skills—both verbal and written—are essential, as is a desire to work closely with a multidisciplinary team.
Most importantly, you’ll be part of the solution to improving healthcare, driven by a passion for saving lives and raising the standard of care for everyone, everywhere.
Responsibilities:
– Deliver virtual telehealth services by conducting video visits to assess patient needs, gather medical history, diagnose, and provide appropriate treatment or guidance.
– Act as a consulting clinician for the Care and Case Management program to help create care plans and support positive outcomes.
– Manage a panel of primary care patients, offering acute, chronic, and preventive care to improve overall health and quality of life.
– Collaborate with a supervising physician as required by state and practice regulations.
– Work with our Credentialing Team to obtain additional state licenses if necessary.
– Maintain patient satisfaction ratings in line with practice standards by providing compassionate, evidence-based care.
– Work from a secure, private home office environment.
– Use office equipment such as computers and peripherals with manual dexterity.
– Handle multiple tasks efficiently in a fast-paced setting.
– Be available for assigned shifts, including evenings and weekends as required.
– Work independently and maintain excellent attendance and punctuality.
– Stay current with medical advancements, treatments, and medications.
– Represent our values through humility and curiosity.
Qualifications:
– Minimum of five years of experience in an outpatient primary care setting (family or internal medicine).
– Proven ability to manage your own patient panel independently.
– Active Nurse Practitioner (NP) license in good standing.
– Current board certification from a recognized body such as ANCC, AANP, APRN, or NCCPA.
– Not currently excluded or sanctioned by any state or federal healthcare programs, including Medicare or Medicaid.
– Must maintain active Medicare status.
– Comfortable treating a wide range of medical conditions, both acute and chronic, as well as providing preventive care.
– Empathetic and supportive, especially when working with patients facing difficult situations.
– Efficient and able to meet volume targets without compromising quality.
– Skilled at prioritizing tasks in a fast-paced environment.
– Committed to maintaining patient privacy and complying with HIPAA regulations.
– Excellent communicator with both patients and colleagues.
– Experience with telemedicine and a high level of comfort with technology is preferred.
Shift Requirements:
– Full-time, 40 hours per week, with 8-hour shifts Monday through Friday.
– One 8-hour weekend shift (Saturday or Sunday) every fourth weekend, with a weekday off that same week to maintain a 40-hour schedule.
– Shifts typically run from 8:00 AM to 5:00 PM, with one evening shift per week extending to 7:00 PM.
